Employee Medical Plan
We provide a comprehensive major-medical health insurance product for our full-time and eligible employees. Our coverage options include individual, you & children, you & spouse, you & family, or no coverage. You may decline coverage.
Dental Plan
Our dental coverage includes Diagnostic and Preventative, Basic, Major and Orthodontia services. Coverage options are identical to the Employee Medical Plan.
Vision Insurance
Our vision coverage includes annual exams, eyeglasses, contacts and is similar to coverage provided with the Medical and Dental Plans.
Life Insurance/Accidental Death and Dismemberment (AD&D) Plan
To help ensure the financial security of those who depend upon you, we offer several options in individual coverage one times your annual pay.
Long-Term Disability
UNUM Life Insurance provides options for financial protection for you and your family in case of serious illness or accidental injury. As a means of providing a regular income, or you may elect no coverage.
Cafeteria Plan
The Cafeteria Plan offered by Chicot Memorial Hospital is pre-tax money for unreimbursed medical expenses, child care expenses, and insurance premiums. The tax liability is decreased by deducting these costs from the employee’s salary before income tax is calculated.
Profit Sharing Plan
The CMH plan sets up separate accounts for each participant. The Plan is operated on a 12 month basis each year beginning April 1 and ends on the following March 31. CMH Board of Governors will decide each year whether it will make a contribution to the Plan, and if the contribution is made and if employees are entitled to a share of that contribution, it will be made to their account.
457 Deferred Compensation Plan
Employee may make contributions (deferrals) from their pay to save for retirement. The amounts contributed will be held in a trust until the employee quits working for the hospital or retires. The contributions are non-taxable when paid to the plan, but become taxable when the employee terminates employment or retires and takes their money.
Work and Family Benefits
In addition to the plans outlined above, Chicot Memorial Hospital offers additional benefit to employees, including the following:
Discounts
CMH employees are given discounts in the Pharmacy, Dietary Department, and Sanders Uniforms (must have PO). In addition, CMH employees and their dependents claimed on their tax return are eligible for a 20 percent on hospital services performed at CMH.
Pharmacy
CMH offers employees the opportunity to purchase OTC and prescription drugs at average wholesale cost plus 10%. Controlled substances are not eligible.
Employee Health
The following injections are offered to employees at no cost: Flu shots, Tetanus, MMR (measles/mumps/rubella), PPD (TB skin test).
Service Awards
Each year the hospital holds its Annual Awards Reception to honor employees who have been employed at CMH for 5, 10, 15, 20, 25, 30, and 35 years.
TELCO Federal Credit Union
The credit union offers to CMH employees savings accounts, checking accounts, ATM card, MasterCard credit card, Mortgage loans, Auto, boat, and RV loans.
Direct Deposit
All employees’ paychecks are deposited directly into your checking and/or savings account.
Check Cashing
Personal checks may be cased in the Dietary Department. A limit of $5.00/day is allowed.
Payroll Deduction
Payroll deduction is available on purchases in the Gift Shop, Pharmacy, Meal tickets, Hospital bills and Uniforms when applicable.
Shift Differentials
Shift differentials are paid for night, weekend and holidays according to job classifications.
Gift Shop
The convenience of payroll deduction adds to the beauty of our hospital’s gift shop, open seven days per week for shopping needs.
Lakeside Athletic Club Membership
CMH has a corporate membership to the Lakeside Athletic Club. Employees can join the Athletic Club at a discounted rate, either individual or family membership.
Paid Time Off (PTO)
Rather than providing employees a specific number of vacation days, holidays and six days, Chicot Memorial Hospital uses an innovative, all encompassing Paid Time Off system. This approach provides for the creation of a "PTO" account for each eligible employee into which the Hospital places accrual hours each pay period. Payment for any time off is made to you from your "PTO" account. The rate at which you accumulate funds in your account is determined by your years of service with the Hospital. Maximum amount to accrue 480 hours.
Other benefits are available to employees and are explained in detail in the HR Department after employment with CMH.
